At Hope Center Houston, we understand that managing chronic conditions, addressing acute illnesses, and receiving preventive care are essential to helping our guests regain stability. Yet for those experiencing homelessness, barriers such as lack of insurance, transportation, and coordinated care often stand in the way. That’s why we’ve developed a medical referral and advocacy program anchored by three key partners:

HCA Northwest Hospital

Our case managers work directly with HCA Northwest to schedule and prioritize outpatient appointments, from post-hospitalization follow-ups to specialist referrals. When a guest is discharged from the ER or inpatient unit, we arrange transportation back to campus and ensure they understand their discharge instructions, prescriptions, and next steps in recovery.

Spring Branch Community Hospital

Through a standing agreement with Spring Branch, we host monthly mobile clinics on our campus, offering free lab work, X-rays, and mental health assessments. Volunteer physicians and nurses from Spring Branch also provide on-site health education workshops—covering topics like wound care, diabetes management, and stress reduction techniques—to empower guests to take charge of their health.

Harris County Health

In partnership with the county’s public health department, we coordinate immunizations (flu, COVID-19, hepatitis), TB screenings, and preventive screenings for blood pressure, cholesterol, and blood sugar. Their team also assists eligible guests in navigating Medicaid enrollment and connects them with substance use treatment resources.
